Tattoo sketch

Working on an arm-piece tattoo idea for a friend, playing off the Peter Pan 'thimble kiss' imagery. I think the line work is getting there, but this first-round of shading seems a little flat. Anyone with a bit more tattoo-drawing experience have any suggestions for how I can make the individual elements stand out a bit better?
