Further Medium cleanups

As per my previous shot, the upgrade button could do with feeling more like an actual button. Added extra affordance, increased the font-weight, used some of the existing interface greys to shade, and added a slight inset border on the avatar so the edge doesn't feel as jarring against the white and juxtaposed with the upgraded Upgrade button.
