Duchenne and Me Version 3

Final design concepts for the version 3 rebranding of the Duchenne and Me app. This will be the third revision to the design and mark the first MAJOR revision to it. This is a large UI refresh but still maintains a consistency in UX to ensure old users don't get lost.
