A minimalist web-app sign up form. I like to keep things simple and only ask for the most important information. Users can add their name and team name when that information becomes important (when inviting other to their team).

The "product updates" checkbox was a reluctant addition, however it's getting an opt-in rate of about 30% without making the UI too busy.
