Daily UI Challenge - 001 - Form

Working on a very long form for an incident. This is actually built on a gravity forms so will need to style the css to look correct. That being said, the simplicity should allow the form to not be too much of a drudge on the users. It also keeps the data clean (no incorrect spellings, etc.).

Any input on cleaning the form design is welcome. My goal is to get better as a designer.
