"Isolated Voices" (Voces aisladas) Book Cover

Mario Chavarría is a guatemalan writer which recently was awarded finalist in a national literary contest. I was commissioned to illustrate and design the cover.
The idea is to portray a character in one of many stories on this book. It is a psychothriller collection of short stories about broken minds.
This is my "directors cut". I really love how the red atmosphere makes it almost unbearable and suffocating.
What do you think? Title design and layout?
