Collaboration on managing diabetes health

I’ve been working on some explorations of different concepts for collaborating on managing diabetes health for patients, clinicians and family members for Tidepool.org.

The intent of this illy is to show the energy of collaboration between a patient and healthcare provider on managing the data collected from diabetes trackers.
