Dashboard feedback

For the first iteration of our dashboards, we have some research data to point us in the direction of certain KPIs to represent, but don't want to dedicate engineering resources to building a fully customizable dashboard.

This solution gives our users a quick way to tell us what's important to them. I tried expanding the form to include more information, but thought it better to leave it as simple as possible, while giving users the opportunity to expand on their feedback in a larger text field if they want to.
