UI design for an emergency relief app

My third HCI/d project - an emergency app for FEMA. We want to create a new channel, an app, that helps people who cannot reach out 911 in an emergency situation, to send out rescue request efficiently. Through this app, users enter their information such as current location, contact info, health status and injured people info. Also, we provide controlled vocabularies for users to mention more details of their situation.
