Call Request Form

A muti-page form that allows a broker to request a call from a client.

Couple of things I had to consider: 1. Multi-page pagination. 2. Progress bar. 3. Inactive/Active state of the buttons depending on the multiple fields' validation. 4. Back button visibility not visible on the first page. 5. Include a visual to the user when creating/saving the fields' inputs to the database. 6. A confirmation that the create action was a success.
