Dashboard: add apps

Hi everyone!

Here's another part of advertising dashboard I'm redesigning.

This is a modal window that appears when user clicks "Add new app". User tests I conducted showed a 2-step process results in higher completion rates than asking for all info on one screen. Users found this flow less overwhelming.

P.S. I often hear phrases like "Let's avoid extra clicks (or extra steps)", and while I think it's true for Mobile with limited screen space it's not an issue on Web anymore. Computers are fast, browsers are responsive, you know.
