Medication Calendar

Less than 25% of patients leaving the hospital understand the follow-up care instructions they’re given.

Patients shouldn’t leave the hospital with pages of medical jargon. They should go home armed with easy-to-understand visual guides emphasizing the information they need.

Too many patients revisit hospitals because they didn't know how to treat themselves. This is a tool that could help people help themselves.

I based this calendar off of a true prescription list. The list was filled with poorly written doctor's notes and long medication names. Even the times were difficult to understand or remember. This visual calendar is meant to communicate the information almost inherently.
