Sign In + Sign Up (Medical app Medicure)

Here are some remaining screens from Medicure app. Sign in and then sign up as doctor or patient and then comes the sign up screen for doctor first two field are filled and others are remaining. Fields are bit long so instead of scroll I think swiping to another screen is a good option for users to see all fields easily and comfortably.
