Population Health Mobile App

This is a design that I worked on that would introduce population health and it's goal oriented functionality into our existing healthcare products platform. I used a mobile first design approach. I thought it would be more relevant to show where you rank among your peers first, then you can see all the rankings as secondary interaction. See full image attached.
