P.O.D Form screen and loading button

A couple of challenges I received from the Daily UI Design challenge were to make a "form" and "button" design.

I came across an article that discussed some great tips for button design. I took these bits of advice and produced a short design that was simple, stripped to the main information needed to make P.O.D (Personalized ordering device). P.O.D is an app used when you would like to order food in a bar or restaurant when the cue is too long or a waiter hasn't come to your table yet.

In addition to this form design, I created a loading state for the button when users sign into the app. The loading button would ideally be rotating when the user presses the button.
