How to ask user for a feedback?

We wanted to come up with a fun and very simple design to ask users for feedback after the relaxation session.

Through gentle animation, smooth colors and music, each session either stimulates and uplifts, or relaxes and calms the body, mind and spirit. The statistics we gather allow us to understand if the therapy is effective, and adjust it in accordance with the feedback.
