Code To A Live Form Presentation

We have a tool at Eligible called Claims.js -- it is a quick JavaScript library that you can drop into your application to save many hours of manual work. The form imitates a "old school" paper form, but also allows instant submission, error checking, pre-filling ..etc. It's magic!

I've been looking for the simplest way to show to our customers that only a few lines of code can generate something incredibly complex, all within the constraints of the doc pages. This was the solution that made the final cut.
