Credit Card Payment Form (Mobile)

Our users can avail of a 14-day free trial of our product. At any point during the trial, they can enter their payment information and select a plan so that they don't lose access when the trial period is finished. On mobile, we shift the credit card part of the form to the bottom (it displays on the right hand side on desktop/tablet).
