Hi everyone!

I'd like to share with you a Booking Form UI for Dental Clinic.

This is a simple and clear one-page form that allows you to book dentist appointments without needing to call or text, which is really useful especially for emergency visits or regular hygiene visits.

I made a quick research and there are not many implemented solutions of such problem, most of the sites have just a simple contact form there you could leave a message and wait for a phone call. I also created a user flow to ensure that the booking process is understandable and logical.
