#DailyUI Day 82: Form

I created an RSVP form for the wedding invitation page I created a few days ago. The form has a white card background to allude to a physical RSVP card.

The first page asks for the guest's name and whether they plan to attend. If they respond with yes, they will get an additional page asking how many people will be in their party, what events they will attend, and which entrees they would like.

The last page asks for a message for the couple and, for those who said they would attend the reception, a song request. Users who said they would not attend would get an alternate version with different prompt text and without the song request.
