Custom Survey Build for Behavioral Health Care Provider
Project Overview
Problem Statement: Facility admins working within the behavioral health industry need a digital tool that integrates with their existing facility management platform and allows them to easily add custom surveys to their assessment pipeline.
This survey builder allows facility admins to easily create their own questionaires and surveys using pre-made question formats including multi-choice (single and multi answer), text / number / date input, range scale, star rating and more. This project was part of a larger assessment-based outcome tracking platform designed for behavioral healthcare facilities.
My Contributions
Role: I joined the team after the research and ideation phase as the UX Designer focusing on Visual Design and Prototyping.
Team: This project was lead by our Director of Technology and included myself as UX Designer, and one Full Stack Developer.
