#DailyUI Day 28: Contact page

On my contact page for my fictional grocery store, the user first selects a contact method: phone call or email. If they chose the phone option, the general customer support number appears. If the user wants to call a local store, they need to choose the location from the list to see its number.

The email form asks for a name, email address, subject topic (dropdown menu), and the message text.
