Online Form Design - UX and UI

A UX and UI Project in which I designed an online registry form for national disease records. I used Figma to create this project. In this form users are allowed to view their infographic data which is generated after entering personal national code, add contact addresses and phone numbers, add family and friends for emergency contacts and add chronic diseases and side effects and also send their data to related official disease communities.
