Connected Care: Track a loved one’s well-being and care

The Connected Care mobile app allows family members distributed across geographies to access the history of the person’s data, such as blood pressure readings and food intake over time.

A health profile page contains the person’s list of health conditions and current medications, so any family member can take their loved one to a medical appointment with all of the necessary information. In addition to the calendar and messaging features, we also built a contact book to centralize care team information.
