Left part: choice of test or procedure, as well as patient data. Database search based on ID. If the patient is not in the database, the fields are filled in manually with the automatic creation of a patient ID.

Central part: choice of date and time of visit.

Right side: summary information about the entry with the ability to send a reminder to the phone via SMS and confirmation of the entry.
