Form in a Modal

Hello Dribbblers!

Here's shot of the design ive been working on from some times.

What this shot about?

This Modal lets the user create a New Checklist Prototype by filling in some basic detials and adding additional authors/collaborators.

Additional authors/collaborators can be viewed as tags and can be easily removed by clicking the remove/close icon inside the tag.
