Mobile App UI for Tracking Polio Cases.

Some of the interfaces I designed for an app meant to track the status of polio in Nigerian communities. With the app, a volunteer can upload information about suspected cases of polio in a particular community to the database and also health workers can access these data and use them for further treatment and immunization.
