Credit card form

We are working on paid user accounts, $10 a year. The benefits include keeping track of your own palettes, creating color samplers, personal trends, etc.

What I wanted to highlight (and get feedback for) from this form is the EXP. DATE. Instead of asking the month and year individually (which I'm pretty sure everyone hates), why not just asking "MM/YY"?

Also, instead of asking for a billing address, we just ask for the billing zip code depending on the country.
