Medical Report App Wireframes

Here's some wireframes for a Medical Report app. The idea is that the app is used by a patient suffering from Alzheimer's disease. I thought it would be a more user friendly to ask simple questions paired with clear buttons to gradually build a report that would then be shared with their HCP (Health Care Professional). The questions could be developed using machine learning to get as clear an idea as possible of a patients medication or symptoms and side effects. A patient would then be able to go back and edit and view their report using the menu.
