Modal iteration

Working on different methods to verify after making a selection between "self-pay" and "covered by provider". One idea was keeping the selection on the page, and using a modal to confirm the selection, and/or fill out the following verification step.

I'm not sold on this yet, just cranking out different methods to see what works the best. Iterate, iterate, iterate.
