Insurance app

A few years ago, I designed an insurance app.
Reviewing today the immediate page after signing up, I confess the mistakes I've done:
x The modal is inaccessible
x No way to close the modal
x It forces the user to do what it asks
x The material design fields are inaccessible
x The colours are inaccessible
