Questionnaire for get credit

Hey, guys!
This shot shows one of the pages of a questionnaire that was developed for get credit in a bank.
To make it easier for the user to fill out the questionnaire, all fields have been divided into categories and groups.
Simple navigation on the left allows the user to see which step he is at and, if necessary, go to the desired stage.
