Synchromedics First Aid

Here is the First Aid screen from my Recent project Synchromedics.

User case: Allow any first responder or company Nurse to record employee injury /illness evaluations and treatment with follow up visits which can be quickly added with date/time stamp and nurse signature and ability to add addendum.
