The nation is in the midst of COVID-19, and it has become apparent that older adult care facilities are in need of assistance.

There have been numerous reports of families unaware of outbreaks within their loved ones facilities, and in some serious cases, no notification of family members being diagnosed with COVID-19

After synthesizing our user research my team and I landed on an app that keeps adult care facilities and loved ones connected. PrimaryCare allows loved ones to view any general facility wide announcements, send messages to their point of contact or the facility, view health and medication logs, events, and most importantly, any incidents regarding their loved one.
