Central Public Hospital Redesign - Online Reservation

Hello folks!

I recently redesigned the central public hospital's online reservation. current website link. I added type of visit to be two where currently only for those who have a medical record, now new patients who don't have a medical record can book online.

My main focus is modern and minimalist design so that all users can use it. I also changed the form to make it more user friendly for example in entering the date of birth.
