WISDOM Checkout - Payment Type

As part of the redesign of the WISDOM restaurant delivery service software, I put together a new checkout flow for customers ordering food online, which is now completely responsive. This shows the credit card info entry form, which shows only after selecting "Credit Card" in the button row above it. The card type will automatically change to the correct provider as the user types in the number (for example, the "37xx" prefix is for American Expres, and "4xxx" is for Visa).
