Naveon Post-Acute Care mobile app

For Naveon’s Post-Acute Care mobile app, we created a personalized experience for Patients receiving post-acute care. The app helps Patients and their Care Teams get to know each other, have important (and sometimes difficult) discussions, and stay connected even after a Patient is discharged.

The "Get to Know Me" questions posed in the app help Care Teams understand their Patients' motivations, goals, and personalities.

Patients can invite family members to use the app, which provides access to content that deepens their understanding of Plans of Care, diagnoses, and services for improved outcomes. Patients are empowered by providing direct feedback on their experiences, while sharing updates with their loved ones from a single source.
