028_Contact Form

Building on Hunt.r for day 28 with this contact form. From within their profile, users would be able to submit a contact form reporting an issue that would open a chat (via email, text or phone) with a customer service agent. This form gives them space to quickly outline their issue so the CS agent can better serve them. As always, feel free to leave feedback in the comments!
